Back Ground Check Process

*Fill in all requested information.
*Read all information, click on "Accept"
*Fill in required payment information.
Cost is $8.00 and you should receive a receipt via email for your payment. You will not be notified as to results, unless they are negative. This is a "no news, good news" process. Department is notified when you have passed or not, but does not receive any reason. Should you not pass, we will notify you and you will be given contact information to clarify the issue.

Reminder for All Stars

Any player who participates in an All Star game, Showcase, or any other baseball program other than his Legion team MUST have a completed, approved Dual Participation form on file. Failure to do so will result in the player's suspension. Click on the link above to get a Dual Participation form #6.
